Job purpose

Our pharmacists work autonomously reviewing prescriptions across a wide range of treatments, medicines and medical devices. Working as part of a team of pharmacists and pharmacy support staff and alongside other experienced managers means you are supported to learn and quickly get up to speed. No two days are the same and our use of Accuracy Checking Technicians means you can focus on the elements of prescriptions and medicines advice that require a pharmacists knowledge experience and expertise.


About the company

Our company is a Clinical Homecare service, providing:

  • pharmacy dispensing and delivery of medicines to patients in their own home

  • nurse training and support of patients to enable them to administer the medicines where

  • Patient Support Programmes that deliver ongoing support to assist patients in concordance

  • with more complex medicine regimes than are traditionally encountered in primary care


Main responsibilities

  • To undertake the role of the Responsible Pharmacist as required ensuring correct records are maintained.

  • To follow and promote relevant SOPs and Company policy and procedure

  • To carry out clinical screening of prescriptions ensuring appropriateness for the patient

  • To provide counselling and advice to patients by telephone, email or in person as required

  • To record interventions made on the electronic patient record as required by the relevant Standard Operating Procedures

  • To understand pharmacovigilance regulatory and contractual requirements.

  • To undertake final accuracy checking efficiently and accurately when required to do so.

  • To ensure that the Homecare pharmacy operation meets all regulatory, contractual, agreed

  • To provide professional supervision of the pharmacy team.

  • To provide advice and guidance to pharmacy colleagues to ensure skills and experience are

  • Undertake pharmacy audits as required.

  • Support the Pharmacy Superintendent and Pharmacy Operations Manager in the

  • To proactively build and maintain effective working relationships with referring centre prescribers, pharmacists, and nurses, taking professional responsibility for the development and enhancement of services provided.

  • To provide professional support and advice about medicines to HealthNet’s nursing team.

  • To carry out any reasonable request given by appropriate persons.

  • Experience

  • Experience in a Homecare Pharmacy setting. (Advantageous)

  • Excellent communication and stakeholder management skills with strong written and oral skills

  • Strong judgement skills and decision-making ability


Qualifications

  • MPharm Degree or equivalent qualification that meets the Criteria for initial registration as a pharmacist with the General Pharmaceutical Council

  • Registration with the General Pharmaceutical Council Desirable

  • Post Graduate diploma in clinical pharmacy (Not essential)
